Cardiology in Family Practice A Practical Guide STEVEN M. HOLLENBERG, MD TRACY WALKER, MD / Acute Coronary Syndromes 1 1 Acute Coronary Syndromes TERMINOLOGY Acute coronary syndromes (ACS) describe the spectrum of disease in patients who present with any constellation of clinical symptoms that are compatible with acute myocardial ischemia. In this sense, they differ from stable angina, which is usually precipitated by increased myocardial oxygen demand (e.g., exertion, fever, tachycardia) with background coronary artery narrowing (limitation of oxygen supply). (See Fig. Classification according to presenting ECG coincides with current treatment strategies because patients presenting with ST elevation benefit from immediate reperfusion and should be treated with thrombolytic therapy or urgent revascularization, whereas fibrinolytic agents are not effective in other patients with ACS. Recently reported studies comparing in-house thrombolysis to hospital transfer for PCI have challenged this notion, however.
